Conditions of enrollment:

• I understand all enrollments are to be completed online.
• I understand I must make my bookings online. Once my booking is confirmed, it can only be adjusted by making a request to the programme coordinator.
• I understand that I must sign in/out, using the tablet or QR code provided, on the days my child has attended.
• I understand that it is my responsibility to escort my child to and from the Starfish Childcare Classrooms and NOT at the gate or car park.
  

Conditions of payment:

• I understand that I will be charged for what I have booked and not what I have attended.
• I understand invoices will be generated one week in advance
• I understand that failure to pay in advance may result in losing my child's spot in the programme.
• I understand that I will settle my account within one week of receiving my generated invoice, unless prior arrangements have been made. Failure to do so, will incur a 5% late payment fee of the balance owing.
• I understand that there is a late pick up fee for every minute my child is picked up late. The rate is 50 cents per minute. This also applies to dropping children off before my booking start time.
• I understand that I need to give at least two working-days' notice for after school care cancellations and absences.
• I understand that I need to give at least one week's notice for holiday programme and special day cancellations and absences.
• I understand that failure to provide the required notice will incur the charges booked.
• I understand that if I am receiving the OSCAR subsidy from WINZ, I will still be charged for the days that I have applied with WINZ.  
• I understand that I must give at least 2 weeks’ notice to withdraw my child from any of the Starfish Childcare Programmes and the Withdrawal From is to be completed.
  

Permission:

• I understand in some circumstances, there may be only one staff rostered on at any of our childcare programmes at any one time.  This will be based on child/adult ratio.  I give consent for any of the Starfish Childcare team to be in SOLE CHARGE in these situations.
• I give permission to use photographs of my child(ren) for promotional and advertising purposes.  These photographs may be used on the Starfish Social Services Facebook page and Website.  If you do not wish to have any photographs to be taken of your child, a written letter or email must be submitted to the Programme Coordinator before the first day your child attending.
• I give permission for Starfish to transport my child when needed, to and from school, to any scheduled outings, and if required due to a medical event needing immediate action.
• I understand that Starfish will receive my written consent to medicate my child if necessary.
• I give permission for Starfish to obtain medical assistance if required.
• I understand that in the event that information on my child is requested, Starfish will gain my consent to release the information.
